Accurate and complete information regarding your insurance must be provided at the time of registration. Your insurance card must be presented at the time of service, as well as a driver’s license per Connecticut State Law. It is the responsibility of the patient to notify the office staff immediately of any changes to the insurance policy.
Statements are provided to patients to submit to insurance companies for reimbursement.
Psychotherapists at the Family Study Center, Inc., do participate with most insurance companies.
Gelinas does not participate with insurance companies. He is an out of network physician.
It is the patient’s responsibility to obtain any necessary proper referrals from Primary Care Physicians for continued treatment when needed. This will be based on each individual insurance plan. Failure to provide proper referrals may cause the patient to be financially responsible for the bill.
Payments, Copayments and Deductibles
Payments for both Dr. Gelinas and the Family Study Center, Inc., are due at the time of service.
It is the patient’s responsibility to determine if their deductible has been met and to notify the office staff.
Self-Pay Payment
If you do not have insurance or choose not to use your insurance, you are considered a “self-pay patient” and are 100% responsible for the services provided to you and your family.
Phone Calls/Cancellations/No Show Policy
Courtesy phone calls are made 24 hours in advance of an appointment with Dr. Gelinas, or on Friday before a Monday appointment, as a reminder to the patient. Please inform the office which phone number is preferred.
Cancellations or changes to appointments must be made at least 24 hours prior to scheduled appointments. Failure to notify the physician/therapist will result in the full charge of that scheduled appointment. Your insurance company will not cover the fee for the missed or broken appointment, and the fee must be paid in full before scheduling another appointment.
Insufficient Funds Check Policy
An additional $25 will be charged to your visit for any checks or payments the office gets returned from the bank due to insufficient funds.
Medications
To remain an active patient of Dr. Gelinas, patients must make appointments within 90 days of their last appointment.
Requests for refills must be made 24 business-hours in advance and may only be called into pharmacies during our published office hours, and not on the weekends, or on holidays.
